ELATION PASSPORT PATIENT PORTAL
Elation Passport is an online patient portal for patients of Member Plus Family Health that provides secure access to communication with your doctor or medical support team from your computer, tablet or mobile smartphone.
[Replies will usually occur within 24 – 48 hours].
By logging in to our portal with the email address you have provided on your Electronic Health Record, (or, if you do not use email and have requested an alternate ID given to you by our staff, with that username and password) you can do any of the following:
- Ask questions of your doctor or medical staff 24/7 from home, work or wherever you have online access
- Furnish information about a non-emergency condition or problem
- View Lab Results after a recent test or examination
- Request a prescription refill
- Request an appointment

How to Get Started:
If you have provided an Email address for your Health Record you will receive an invitation to login using that information.
If you have not provided an email address to Member Plus Family Health, speak with a member of our staff to set one up.
If you do not use email, you may request an alternate ID for logging onto Elation Passport.
If you are experiencing trouble logging in to your Elation Passport account or are experiencing other technical issues, please complete an Incident Report.
